HOW TO PARTICIPATE:
1. Pick your team of Six Players. Choose a Team Name and Team Uniform.
2. Fill out the Team Form and Waivers (every player must have a parent signed waiver to participate)
3. Turn in your team form, waivers, and payment at the front office or on the day of the tournament.
4. Each team costs $60 ($10/ player)
