Clearance Forms

  All students (including eighth-grade students entering high school in the fall) must be cleared to participate in any sport, band, cheer/song, dance team and vocal ensemble/choir. 
Clearance requires completion of the following items.  Please see below for an explanation of each one:
1.  2024-2025 Athletic Registration (New Online- Home Campus)


Your student WILL NOT be allowed to try-out or participate until they are cleared.  


1.  Online Athletic Registration:

To complete the online athletic registration, please print the "Home Campus Registration Instructions" below and then click on this link:

A copy of the student's medical insurance card (front and back) and the DMV Risk Management form are REQUIRED to upload into the athletic registration. You cannot participate without health insurance.  If you do not have insurance, you can obtain it for school athletics through Myers-Stevens. To enroll for Myers-Stevens insurance, click here:

The DMV form is a downloadable pdf found under the "files" section of the athletic registration. This form allows students/parents to drive to or from sporting events when not using District Transportation.  After you fill it out and sign it, take a photo of it completed and upload it in the files section. Once you have submitted the online registration, you should receive an email confirming the registration and a notification will be sent to the school.  

 2.  Physical  (turn in to school front office):


THE PHYSICAL MUST BE DONE ON THE DISTRICT FORM (see pdf file below) or it will not be accepted.  Do not turn the physical in to the coach. Physicals are good for one year from the date of the physical.  If you think your physical is current, please contact Mrs. Belleville and let her know (contact info below).
